Menu Not Focussed

I have a menu, for example...

And randomly (maybe 25% of the time), it sometimes doesn't get focus, which means up/down won't work, and the hotkey isn't underlined. You have to click the item you want with the mouse, even to close the menu (because it can never get a blur if it's not focussed).

I've been using this same script for many years now, and it's never been a problem... only recently. Any idea what could be causing this behaviour?
